These notes are designed to help you complete the application form and gather the necessary evidence to support your application for the WaterSure scheme.
Guidance notes for applicants
Benefits or tax credits
You should have received a notice of entitlement or award notice when you first applied for benefit/tax credit. It is important that you enclose a photocopy of your latest entitlement or award notice. If you have been claiming benefit/tax credits for over a year, you will have received an annual notice telling you the level of benefit for the current year. You will also have received a notice if your circumstances have changed. You must make sure that the notice is less than one year old (less than six months old in the case of tax credits) and that it gives your current address.
Applicants with three or more children
If you are claiming because you receive child benefit for three or more children under 19 (living at the same address and in full-time education) you will need to enclose evidence of entitlement to child credit and the dates of birth. This should be a completed copy of form DL84 TS available from the Child Benefit Centre.
Applicants with specified medical conditions
If you are claiming because you or a member of your household has one of the specified medical conditions listed on the flowchart, you will need to have the relevant section of the application form completed and stamped by your consultant, GP or health visitor.
Applicants with medical condition other than those specified
If you are claiming because you or a member of your household has a medical condition, which results in the use of significant amounts of water for essential purposes, you will need to provide a signed certificate issued by a doctor or registered practitioner along with your application form.
Evidence required to support your application
Depending on your circumstances, you should include copies of the following documentation with your completed application form:
Benefits or tax credits
Photocopy of the latest notice of entitlement or award notice
Less than one year old for benefits
Less than six months old for tax credits
If you have lost your notice of entitlement or award notice certificate, please get a replacement certificate by contacting the organisation that administers the benefit or tax credit to you.
Three or more children
Evidence of the entitlement for child benefit, which includes dates of birth, this should be:
A completed copy of form DL84 TS available from the Child Benefit Centre
Medical condition (as specified on the flowchart)
Application form completed and signed by either:
Consultant
Health visitor
GP
Other medical condition
A written certificate / statement signed by a registered medical practitioner, which states:
The name of the person with the medical condition
The diagnosis of the medical condition, which results in significant use of water
Date the certificate was given
Name and address of the registered practitioner/Doctor
It is essential that you include the appropriate documentation as we are unable to accept you on the scheme without it.
